19 year old male came to Opd with chief complaint of fever sinces 3 days

I have been given this case to solve in an attempt to understand the topic of " patient clinical data analysis" to develop my competency in reading and comprehending clinical data including history, clinical findings, investigations and come up with diagnosis and treatment plan


The patient/ attender was informed the purpose of the information being acquired. An informed consent was taken from patient/ attender and there is omission of information that was requested to be omitt.

Chief complaints:
A 19 year old male came to Opd with chief complaint of fever
Patient went to local hospital for treatment of fever 
And he diagnosed of dengue fever then he came to Opd

History of present illness

patient was asymptomatic 3days back. Then  he develop fever associated with headache, eyepain, fever intermittent Spike of temperature every 2 hours reduced on taking medication, fever is associated with join pains, chills.

and 1 episode of vomiting (non billius) 2 days back
Past history:-

No similar complaints in the past

Not a know case of DM,ASTHMA,HTN,EPILEPSY,TB

Diet:mixed
Appetite:normal
Bowel and bladder movements:normal
Appetite : decreased
Sleep       : adequate 
No additions

No diabetes and hypertension

On examination:-

Pt is C/C/C well oriented to Time ,place and person 

No pallor,icterus,cyanosis,clubbing,or generalised lymphadenopathy 

febrile
